Mama Llama Lactation is happy to now offer OHIP-subsidi infant feeding support (breast/chest feeding, formula feeding, starting solids, etc) as part of the Ontario Breastfeeding Clinic in partnership with the OBCPG.

Step 2:
Have your DOCTOR/MIDWIFE/NP fill out the REFERRAL FORM
Our referral form can be found on the Ontario Breastfeeding Clinic Website. Your care provider can print, fill out and fax these forms, or fill them out and submit them online.
If you have difficulty getting a referral from your care provider, please click here to reach an online clinic where a lactation referral can be submitted for you.

Complete your intake form
Please complete the intake form that will be emailed to you prior to your appointment. Appointments cannot be booked without a completed intake form.

Question: Do I need a referral from my primary care provider to book an appointment? Answer: Yes, referrals are required to book an appointment. You can get a referral from your OB/Midwife/Family Doctor or Nurse Practioner.
Question: Can you help with pumping, nipple pain, tongue tie, low milk supply and more? Answer: Yes! I have experience working with many moms that have these concerns. A little bit of help can go a long way!
Question: How can I prepare for a virtual appointment? Answer: Prior to your appointment, you will be sent an email with a link to join the virtual appointment. Please download the app beforehand so you are ready to go at the time of your appointment to ensure we can make the most of our time together. During an appointment, I like to observe a feed to be able to better assist you. If you have a laptop or have a helper that can hold the camera during the feed, that can help me see your baby's latch better.
Question: I filled out the form to book an appointent, but I haven't received an appointment time yet. What should I do? Answer: Please check the email address that you provided on the "book now" form for an intake form. An appointment cannot be made without submitting an intake form.
Question: My initial feeding concerns have resolved, but I have some new feeding concerns. Can I book another appointment? Answer: Yes, you can book another appointment. Please have your health care provider submit a new referral form for new issues/concerns that arise.
